4. Review & human approvalA named reviewer checks drafts, citations, and confidence; low-confidence, ambiguous, ungrounded, or incomplete questions are blocked or routed by name.

Grounding & gate rules

Drafts cite the doc title + section they came from and carry a confidence score; ungrounded questions get no answer and are routed to a human by name — a failed lookup never invents results. Question text missing is blocked by name; low-confidence drafts (under 70%) need human review before any CRM write; the same question from the same requester can never enter twice; and every CRM write needs a named human approval. The audit trail shows who approved what, and when.
